NAVIGATING (MOVING THROUGH) THE FORM

Before submitting, you must review the form. Submission is only possible once all required fields are completed and there are no errors. After submission, no changes or further uploads are allowed. A confirmation email with a copy of the form will be sent—if you don’t receive one, your form has not been submitted.
Attachments must be uploaded from your device, allowing time for each upload. Files can be up to 25MB, though smaller files are recommended. Teams can collaborate using the same login (or Smartyfile), but only one person should work at a time.
